Youth Forms:  All players must fill out and return copies of the registration form, financial responsibility form, health form, consent to treat form, and the Nighthawks waiver. Additional waivers may be required during the season for tournaments or other activities. Failure to fill out and return the required paperwork may result in player being banned from team practices and/or games.
